Dental Insurance

We understand that dealing with insurance can be stressful. That’s why our knowledgeable team will track, file, and update your claims for you. We’ll do everything we can to maximize your benefits and minimize your out-of-pocket costs so that you can get the most out of your coverage. We accept most major insurances providers both in and out-of-network, Medicaid, Medicare etc. We also accept Flex Spending Accounts (FSAs) if set up by an employer and Health Savings Accounts (HSAs) if enrolled in a compatible health plan. If you have any questions regarding your insurance, our team is fully trained to assist you!

Next Level Smiles Savers Plan

If you’re uninsured, you can still save money on your smile needs with our Next Level Smiles Savers Plan. For one annual fee, you‘ll receive full preventive care and all necessary X-rays at no additional cost. You’ll also receive discounts on other treatments you receive at Next Level Smiles. Membership Details

Patient portion of bill is due at time of service
Cannot be used in conjunction with another dental plan or dental insurance
No refunds of premiums will be issued if participant decides not utilize dental plan
If participant opts out of plan in writing within 30 days, a full premium refund will be given
No membership card will be given
Your plan’s effective date will be on file with our office
Plan cannot be used for specialist referrals or hospital care
Plan cannot be used for costs of dental care which are covered under automobile medical insurance
Plan cannot be used for injuries covered under worker’s compensation claim
Plan cannot be used for treatment which, in the sole opinion of our doctors, lies outside the realm of their capability
Office fee schedule subject to change
Family members may be added for an additional $110 per year.
Dual plans are applicable for a parent and child duo or two spouses.
Children eligible for the family plan include those under the age of 18 or full-time students under the age of 23.
